CASE MANAGEMENT / TRAJECTORY COUNSELING within a LOcal ASSISTED-LIVING care network
Through the Case Management / Trajectory Counseling Project we wish to further develop the methodology within a local assisted-living care network, in order to allow vulnerable elderly people to live at home for as long as possible and in a qualitative manner.
This project aims, in a socially and economically responsible manner, to provide an approach that offers a concrete response to the current and future problems of the elderly, based on quality, effectiveness, affordability and sustainability of care for the elderly. Quality and effectiveness of care and welfare services increase when the case management/trajectory counseling methodology is applied, provided the development of a complete care plan or individual care trajectory includes the different life aspects and life trajectory of the client, and if it is executed in cooperation with a local, integrated, (in)formal care network. The intention is to achieve affordable and sustainable care through the efficient and effective use of care personnel.
The project is based around the ability of the elderly person to live alone, and develops around and with this elderly person and his/her surroundings a trajectory of seamless care in which informal carers, volunteers and professionals work together. The project intends to develop an innovative approach, whereby the vulnerable elderly person living longer at home is achieved, starting from the concrete problems of the elderly person him/herself, and utilizing the possibilities of this elderly person and his/her environment, supplemented by the effort of local volunteers and professionals.
